General Policies

  1. All patients that are coming in for a procedure that requires anesthesia will need to be up to date on all core vaccines (canine and feline) and heartworm testing (canine only) for the safety of the patient.
  2. Kindly give us at least 24 hours' notice if needing to cancel or reschedule a surgical appointment. Short notice cancellations for surgical appointments will be incorporated into the no-call/no-show strike policy. (See below)
  3. If you arrive more than 15 minutes late for a scheduled appointment, you may be asked to reschedule or have a longer wait time.
  4. If you arrive more than 15 minutes early for your appointment, you may be asked to wait until your scheduled appointment time.
  5. There will be a walk-in fee for any unscheduled appointments that we are able to accommodate. Any walk-ins that we cannot entertain will be denied and referred or scheduled appropriately.
  6. Payment is due in full at the time of service. We do offer Care Credit as an option; you can find more information and the link to apply on our Pet Insurances page.
  7. Fremont Animal Hospital has a zero tolerance policy for disrespectful inappropriate behavior of a client to or staff, doctors, and clinic policies. If a client is disrespectful, the client will receive a behavior warning letter or a termination of the veterinary client patient relationship letter. Please be kind.

New Patient Policies

  1. We are currently accepting new clients!
  2. New clients must pay a deposit when scheduling their first appointment.
  3. You must give 24 hours notice to cancel your appointment to get your deposit refunded.
Please note: If you go three years without bringing a pet in to be seen, you will no longer be considered a client.

Urgent and Emergency Care Policy

Emergency and urgent care services are only available for active clients whose pets we have examined within the past year. If we have not seen your pet in the past year, we cannot provide urgent or emergency care for them. We will schedule appointments for pets we’ve seen within the last three years and may require a deposit.

Rabies Policy for Drop-Off Appointments

Your pet must be current on their rabies vaccine to be eligible for drop off appointments, technician toe nail trims, or technician anal gland expression appointments.

NO CALL/NO-SHOW Strike Policy

Appointments that are missed, canceled, or rescheduled without at least 24 hours notice will result in a strike against the clients account.

Client Notice: Patient Controlled Substance Prescriptions

As of January 1, 2022, Fremont Animal Hospital (FAH) will be prescribing all long-term controlled substance medications to a human pharmacy of your choosing. Due to DEA regulations of controlled substances, the veterinarians here at FAH will only be dispensing a maximum of three days of controlled substance medication(s) for your pet(s). Please be prepared to complete a "Preferred Pharmacy" form to keep on file for future use.
